Path Health is founded by Ayana Herbert, MD—a Board-Certified Emergency Medicine Physician with 20 years of experience on the front line. In the ER, physicians often witness the consequences of untreated or poorly managed chronic conditions, including some of the leading causes of early mortality such as heart disease, stroke and diabetes. By opening a health and wellness practice and following the principles of Lifestyle Medicine, Dr. Herbert and her team of trained providers are in the position to adopt and effectively apply a more holistic approach, addressing not only minor physical ailments but also mental, emotional and spiritual aspects of health—all while providing continuity of care and personalized support over time.
Path Health has the goal of filling the gap of access to preventative care and empowering patients with knowledge about healthy living, disease prevention, and self-care strategies, ultimately contributing to improved public health outcomes.
