Client-Centered Nutrition

A client-centered nutrition plan is a personalized approach to dietary management that prioritizes the individual needs, preferences, and goals of the client. Here’s how a client-centered nutrition plan is typically developed and implemented:

The first step at Path Health begins with a thorough assessment of our client’s medical history, current health status, dietary habits, lifestyle factors, cultural preferences, and nutritional needs. We then expand our assessment with a dietary recall or food diary to gather additional relevant information. Based on the assessment findings and the client’s preferences and goals, specific dietary goals and priorities are established collaboratively between the Path Health provider and the client. Goals may include achieving or maintaining a healthy weight, improving nutritional deficiencies, optimizing athletic performance, or addressing food intolerances or allergies. The nutrition plan is customized to meet the individual needs and preferences of the client while aligning with their dietary goals and priorities.

Recommendations may include specific dietary modifications, portion control strategies, meal planning guidance, recipe suggestions, and practical tips for grocery shopping, cooking, and dining out. Path Health’s team of providers empower clients on their journey with knowledge about the importance of nutrition, the role of different nutrients in the body, and the potential impact of dietary choices on health outcomes. The nutrition plan is dynamic and evolves over time based on the patient’s progress, feedback, changing health status, and life circumstances. Regular re-assessment of dietary intake, nutritional status, and adherence to the plan allows Path Health team to identify areas for improvement and make necessary adjustments to optimize outcomes.
